Surgical video demonstrating pars plana vitrectomy for a combined retinal detachment with a full-thickness macular hole. Following meticulous vitrectomy and vitreous base shaving, selected peripheral tractional membranes were preserved to avoid iatrogenic retinal breaks. The macular hole was repaired using a temporal inverted ILM flap, with silicone oil tamponade. Following oil removal, the retina remained attached with successful hole closure and good visual recovery.

Autologous Retinal Transplantation Using a Retinotomy Edge Graft for Macular Hole During Complex PVR Retinal Detachment Repair

This video demonstrates autologous retinal transplantation (ART) for closure of a full-thickness macular hole during repair of a complex proliferative vitreoretinopathy (PVR) retinal detachment. The graft is harvested directly from the relaxing retinotomy edge, avoiding a separate donor site. Medium-term perfluoro-n-octane tamponade and adjunctive serial intravitreal methotrexate are used to optimize retinal reattachment and reduce recurrent PVR.

Subretinal fluid improves retinal mobility and facilitates centripetal approximation of the hole edges, while the ILM flap provides a scaffold for glial proliferation and tissue repair. This synergistic approach may be useful in large, chronic, persistent, or recurrent macular holes.
