Intraoperative Fluorescein Guided Retinal Vein Occlusion Sheathotomy
This is a patient with a chronic ischemic RVO, dense vitreous hemorrhage, and tractional retinal detachment. The fibrovascular membranes were well visualized with intraoperative FA which permitted efficient membrane delamination. Once this was achieved and the vitreous hemorrhage was cleared, we found a sclerotic superior retinal branch vein with a fairly large area of ischemia. We re-injected fluorescein to guide a sheathotomy which re-established blood flow and achieved a 20/30 result.
