Modified Yamane 3-piece AR40 Scleral Fixation with small pupil facilitated by iris hooks

IOL removal and secondary IOL scleral fixation in a 93 y/o female with a dropped IOL/capsule with extensive zonular laxity, IFIS, and poor dilation. The use of iris hooks made this case straight forward with a centred IOL and excellent visual outcome. The video highlights the importance of 1) Even marking for scleral needle entry sites to ensure a centred IOL 2) Intraop trouble shooting when dealing with a small pupil 3) surgical maneuvers to rescue a missed haptics during scleral fixation.
