Four IOLs, One Hero: A Yamane Success Stoy.
A 76-year-old male presented with a dislocated 3-piece intraocular lens (IOL) and baseline iridodonesis. We present a challenging surgical case that highlights the complexity of real-time decision-making and the potential for multiple unforeseen intraoperative complications. The original IOL was easily explanted. Three successive lenses were then inserted and explanted due to delivery failure, fracture, IOL tilt, and needle disinsertion-all while managing a floppy iris. POD#1 BCVA was 20/25.
