Diabetic vitrectomy remains one of the most challenging yet rewarding surgeries in vitreoretinal practice. In this video, we present a case of juxtamacular fibrovascular proliferative membrane (FVPM) causing focal traction and risk to the macula. Key surgical considerations include fellow eye status and visual acuity. Timely intervention with small-gauge vitrectomy (25G/27G) allows safer dissection through narrow tissue planes, reducing the risk of iatrogenic breaks and vitreous hemorrhage. Earl
